As a member of the PTA at my daughter's school we used Occasions by Melody to create bookmarks for the entire school as a going away gift for the students.  These custom bookmarks were unique for each class.  Melody's services were top notch.  She made it easy for us and we didn't have to worry about a thing.  We were all very satisfied with the final product.  I would highly recommend using Occasions by Melody for any of your custom needs.
